As a pharmacist, most of us want to cry when we see insulin prices too. The pharmacy keeps very little of the copay. Pharmacy benefits management companies do what are called “claw backs” where they only leave what they determine to be the pharmacy’s cost for the medication and a nominal dispensing fee, usually less than $5.00. The insurance companies are making money from both sides.
